Status Reports must be filed within 12 months after funding of the grant (only for any grants more than $5,000) within 60 days after the project is completed, whichever is sooner. The Foundation will not accept applications from a grant recipient which has failed to file timely status reports for a previous grant. Grants of $5,000 and under do not require a status report. If this report is required, we strongly encourage recipients to complete and send (via email to firstname.lastname@example.org) the Final Grant Status Report as soon as funds are expended. If you email this form, you do not need to also send in a hard copy.
